Merchant Card Enrollment Documents/Forms - General Info

On February 1, 2015, the Office of the State Controller executed a new Master Services Agreement (MSA) with SunTrust Merchant Services. Following is information and instructions for eligible entities desiring to enroll as a participant under the contract. Information and forms are also available for current participants desiring to add additional outlets or applications, or to conduct certain maintenance functions.

All Merchant Card forms are completed through DocuSign.  Scanned, faxed or paper copies will not be accepted.  If you have questions about the forms please contact PDF versions are for informational purposes only.

Requests through SunTrust Merchant Services take approximately 7-10 business days to complete. Please consider this when submitting request forms. POS equipment can be expedited, but an extra fee will be incurred.
