NCFS Bank Account Authorization Form

The NCFS Bank Account Authorization form is intended for additions, changes or deletion of bank accounts in NCFS. The Department of State Treasurer is responsible for submitting the form for all accounts owned by DST.  Please work directly with DST to begin the process of establishing, changing or disabling a DST-owned bank account.  For Non-DST bank accounts, the form should be fully completed and signed before emailing it to the NCFS Help Desk at NCFS@osc.nc.gov.  An NCFS team member will reach out once the request has been completed or if there are questions.
