SUBJECT:	Self-Assessment of Internal Controls

Please be advised that there are no revisions to the internal control questionnaire for the 2000 fiscal year from the 1999 fiscal year questionnaire.  Please download the questionnaire and respond to the questions as appropriate for your organization.  The internal control questionnaire is a management tool to be used as an assessment of an entity’s internal control system.  The questionnaire should be reviewed for the fiscal year ended June 30, 2000.  Any changes should be noted on the existing questionnaire.  A new questionnaire does not need to be completed for each fiscal year.  A complete copy of the self-assessment of internal controls can be located as follows on the OSC website:
