| SUBJECT: | Direct Deposit Envelopes and New Reports on X/PTR |
We previously sent out notification
that the direct deposit forms were changing to a new size which is 8 ½"
X 11". This new direct deposit form can be folded one time and inserted
into an envelope, which was used for the employee's W-2 form.
If your agency wishes to use this size envelope, you should obtain a quote
on this DW-387 security double window envelope; copy attached, WITHOUT
the imprinted words - "Important Tax Document Enclosed". The
size of the DW-387 envelope that fits the newly designed direct deposit
form is 5 5/8" X 9". The direct deposit may also be folded two times
to fit into the number 10 single window envelope available from Prison
Enterprise. The window is on the left. We have listed below the names
of the suppliers of the DW-387 double window security envelopes.
Your agency should contact these suppliers to obtain a price quote based
on the quantity needed.

The letters "S" and "D" cannot be used in the subhead. Any subhead containing an "S" must be changed immediately. Subheads containing a "D" will need to be changed before the end of December. Remember that these changes will need to be made through State Personnel. A report is available on X/PTR to help identify these subheads:
