| SUBJECT: | Payroll Transmittal Modification for June 2001 |
We have deleted the premium deduction amounts pertaining to the WellPath Select Health Plan from the monthly payroll transmittal. Our decision to make this modification was based upon two notifications, copies attached. The first notification was mailed to members enrolled under the WellPath Select Health Plan providing notification that their coverage would no longer be offered effective July 1, 2001. The second notification was from Jack W. Walker, Ph.D., Executive Administrator of The Teachers and State Employees' Comprehensive Major Medical Plan, requesting that your agency transition applicable employees to the State Health Plan effective July 1, 2001.
Our deletion modifications were to the deduction codes, (470-473, 870-873) on the monthly transmittal, see listing attached if applicable. If there are any deductions for WellPath Select on cycles other than cycle 'A' they will need to be handled manually by your agency.
Please notify your employees that they MUST fill out an Enrollment Form and return this form back to your office according to the deadline specified by your office.
