| SUBJECT: | State Health Plan Premium Increases |
The September monthly payroll transmittal, cycle ‘A’, has been updated to include the new rates for the State Health Plan. We have also changed the ‘800’ series codes back to their respective ‘400’ series codes and provided a listing, if applicable. Any employee who wishes to reject the deferred status will need to once again fill out a new rejection form. We only updated the insurance premiums for cycle ‘A’. All other payroll cycles will need to be updated manually by your agency.
