| SUBJECT: | Notice of Deposit Form Change |
The NOTICE OF DEPOSIT form
will soon be changing from the present 3 ½" X 7 ½" form to
the larger 8 ½ " X 11" size. The change in size will enable
us to provide additional information. Please find attached a copy
of the large form. Please note the following:
| 1. | The form prints your agency
name and mailing address. The default name and address is the
is that of Central Payroll. We will be establishing a table for the actual mailing address to be used for each agency. |
| 2. | The form now prints the three-digit agency identifier, budget code, company, account and center from the Payroll Master File. The information is shown above the employee's name. |
| 3. | The employee's name and address is pulled from the Payroll Master File. Note that the information is shown twice and that there are two locations. This positioning will allow for the use of either of two different double window mailer envelopes. The double window envelopes can either be a #10 envelope or an envelope similar to the double window envelope used for the W-2 statement. |
| 4. | Please note the breakdown in the middle of the page on the right to identify the type of pay being given the employee. |
| 5. | There are now four lines available for providing a message to the employee. In the past we were limited to the use of only two lines. |
| 6. | The description for section 125/129 now has a (+) indicator. This indicator denotes deductions, which are PRE-TAX deductions. |
| 7. | The other non-deferred deductions has a (-) indicator. This indicator denotes other deferred deductions. |
| 8. | In the box headed Miscellaneous Deductions you should note an indicator on the left side of the deductions. If the indicator is a 'blank', the deduction is not tax deferred. A (+) indicator indicates a section 125/129 PRE-TAX deduction. A (-) indicator denotes an other deferred deduction. |
| 9. | At the lower left side of the form note that additional information will be available, if there is weighted premium pay. For Shift Pay the percentage, hours and pay amount will be displayed for those fields having activity. The other pay types O/T, S/T, HOL and O/C will display the hours and pay amount, if any. |
Due to the size of the new form, it will be necessary that new envelopes be ordered, if you are currently inserting the forms. We will provide additional information about the envelope suppliers later this month
In October we will produce both the small and large NOTICE OF DEPOSIT form for the employee. Only the large NOTICE OF DEPOSIT form will be produced at the end of November and thereafter.
We need the mailing address for your agency to be used on the NOTICE OF DEPOSIT. Please make a copy of this page and provide the information below. The information may be faxed to the attention of Ms. Carol Stephenson at (919) 981-5570 on or before September 20, 2001.
