Please find attached a revised North Carolina Employer Profile and Instruction sheet for the National College Savings Program. We are sending this form to you, the PAYROLL OFFICER, and ask that you fill out the payroll information requested in Section Number two. We also request that you deliver this form to your agency Personnel Section. The Personnel Section should supply the information requested in Section Number one and then sign Section Number four. If there are any questions, a quick review of the instruction sheet should provide the answers. Upon completion, the form should be returned to the College Foundation address as listed on the instruction sheet by March 31st.
