User Support Record Form

The User Support Record Form is used to record North Carolina Accounting System events and their resolution whenever a client agency reports a serious problem or makes a request for Client Support services. All requests are tracked at OSC Client Support by entering a User Support Record into the Change/1 event tracking system. The Change/1 system resides on the OSC local area network.

User Support Records may be:
 

1) submitted directly by the client agency using the User Support Record form, 
2) completed by Client Support in response to an agency phone request, or 
3) originated by OSC staff to request system changes.
